The system forms repository will house a variety of forms used throughout the Office of the Chancellor and the Minnesota State system. Unless otherwise indicated, all forms are in Adobe PDF format.

Photographic subject consent form
This consent form has been provided to ensure that any person being photographed for a print or Web publication has provided the Minnesota State Colleges and Universities system with the legal consent to use his or her image.
